CURRENT 2025 USCIS 693 UPDATES:
1. New COVID regulation: As of Jan 22, 2025, covid vaccines are no longer required by the USCIS.
“As of January 22, 2025, USCIS no longer requires applicants to provide documentation of COVID-19 vaccination on Form I-693. This means that USCIS will not issue a Request for Evidence (RFE) or Notice of Intent to Deny (NOID) for applicants who don't provide documentation of COVID-19 vaccination. “
2. As of May 1, 2024, proof of childhood POLIO VAX complete series is required. If there is no proof, a POLIO booster vaccine (iPOL) is required. There is no useful blood test for polio immunity.

USCIS 693 Exam Doctor's office fee: $275.00 (cash, check or credit card) You can receive an official MD receipt for your insurance upon request.
IMPORTANT: To avoid any misunderstanding: You and /or your insurance company are 100% responsible for any lab test, chest x-ray or any vaccine. Those costs are NOT part of the 275.00 office fee.
Note: You can possibly save $300.00 to $400.00 dollars because some vaccines can be FREE or low cost. Please wait until after your test results are complete to organize any vaccines. This is faster and less complicated.

Please PRE register on line. If needed, you can type in “none” or “999999” for some fields. Or use dummy images.

We will collect a blood sample (and a urine sample if you are between 18 - 25 years of age). Fasting is not required.
Test results return after 4 or 5 days.
After we have all your test results, we will know if any additional vacines are required. After you complete all required vaccines (and a chest x- ray only if needed), you can sign and take away your official, sealed USCIS 693 envelope.
FAQs:
1. You do not need a second MMR vaccine to complete the USCIS paperwork.
2. You do not need to bring a copy of a blank USCIS 693 form.
3. You may receive a bill / invoice from Sunrise Labs or another lab if you do or do not have insurance. This LAB bill is 100% your responsibility.

5. If you are 18 -25 years old, USCIS requires urine sample at the time of your visit. Urine sample not required for persons over 25 years of age.
6. USCIS now requires a POLIO booster if you do not have childhood record of the complete series. This is a RECENT requirement.
7. Can I have my labs done in advance and ordered by another doctor? No. The USCIS requires that a Civil Surgeon (me) takes the blood sample and orders all lab tests.
8. It is recommended that you do not get any vaccinations before we have all your test results.
9. Proof of Covid vaccinations are no longer required.
VACCINE EXPLANATIONS:
Each vaccine has a different story. To complete the 693 process, ADULTS need proof of immunity OR new vaccination for MMR, TDAP, Varicella, Hepatitis B, FLU vaccine (only during flu season) and maybe a Polio booster. Children need to complete all required vaccinations for their age.

The Polio story: There are 2 ways to satisfy the 693 Polio requirement. Proof of having had the complete Polio series as child (4 or 5 doses) Or a new Polio vaccine (iPOL) booster dose.
*You do not need the complete the full Polio series to complete your 693 form. ONE new Polio (iPOL) vaccination is adequate to complete the 693 process.
The MMR story: Measles, Mumps, Rubella. This vaccine is three different vaccinations in one single dose. There are 3 ways to satisfy the MMR requirement. Immunity to all three viruses by a blood test, proof of having had two (2) of these vaccines in your past or one new MMR vaccine dose.
*You do not need the complete MMR series to complete your 693 form. One new MMR vaccination (or serum immunity) is adequate to complete the 693 form.
The Varicella story: Varicella or “chickenpox” is also a virus. Usually of childhood. There are 3 ways to satisfy the 693 Varicella requirement. Immunity to Varicella by a blood test (serum immunity), proof of having had 2 of these vaccines in your past OR one new Varicella vaccine booster dose.
You do not need the complete Varicella series to complete your 693 form. ONE vaccination of Varivax (or serum immunity) is adequate to complete the 693 process.
Hepatitis B: This is a virus that infects and damages the liver. It is contagious by blood to blood via: infected transfusion, infected needles, sexual contact or infected tissue transplants. There are 3 ways to satisfy the 693 Hepatitis B requirement. Immunity to Hepatitis B proven by a blood test (serum immunity), proof of 2 Hep B vaccines or one new Hepatitis B vaccine booster.
You do not need the complete series of Hepatitis B to complete your 693 form. ONE new Hepatitis vaccination (or serum immunity) is adequate to complete the 693.
The Flu Vaccine story: USCIS requires the annual FLU vaccine from October 1st until March 31st. In other months, the FLU vaccine is NOT required.
